The X12 837 EDI file format allows for more information than fits on a paper form. The creators of the 837 standard included many health care business scenarios besides only a professional or institutional environment. Here is a sample of an 837 form with these segments:

In addition to the standard UB-04 or CMS-1500 forms, HIPAA Claim Master has the option to include Additional Information and/or Coordination of Benefits (COB) information. The application checks each claim for the existence of those special segments. Then, if this additional data is in the claim, the application will display it.
To include the Additional Information and/or COB when printing out forms, make sure you check these settings in the application by going to Setup > Options in the main menu window. If you only need the regular claim form without the additional pages, leave these boxes unchecked:
oAdditional Info — Check this box to print extra information added to the regular form.
oCOB Info — Check this box to print any COB information added to the regular form.
